Who may be eligible:
If a person has a medical condition that requires treatment or rehabilitative services prescribed by a physician, as well as a physical and/or mental impairment that hampers activities of daily living but is not severe enough to require 24-hour institutional care; and if there is a reasonable expectation that preventative services will maintain or improve the present level of functioning, along with a high chance of further deterioration and probable institutionalization if ADHC services were not available.

What you need to apply:
- Complete the Golden State ADHC application.
- The application that is required includes a form for the applicant and a form for the applicant’s physician. Download and Print the application.
- A recent TB test must be completed for your application.

The entire program cost may be covered by Medi-Cal Managed Care Health Plan. Participants who are not covered by Medi-Cal may enroll with customized charge as a (Private Pay).
