About Us
Golden State Center is a licensed day health care program by the California Department of Public Health that provides a combination of medical, social, and therapy services to adults who have difficulty functioning in their own homes.
Our highly qualified staff can provide a structured day program which includes nursing, meals, transportation, social services, and restorative therapies such as physical, occupational, and speech therapies, in a warm, caring, and secure environment.
Our Mission
Golden State provides medical, rehabilitative, and social services to elderly persons and other adults with functional impairment, either physical or mental, for the purpose of restoring or maintaining optimal capacity for self-care. Golden State will provide top-quality services through an individual plan of care and target rapid growing number of adults who are at the institutional level of care or at risk of institutional placement. Golden State is a Medi-Cal benefit program and is administered by California Department of Aging (CDA) and licensed by the California Department of Public Health as a health care facility. Golden State is an approved Community Based Adult Services (CBAS) provider.
Professional Staff
Our Center’s competitive edge is the quality of the licensed professional staff as well as the modern facility to provide the best possible care. Our diverse staff members will care about the health, safety, and well being of each of the participants. Our team will include multidisciplinary professionals such as Registered Nurses, Speech Therapists, Occupational Therapists, Physical Therapists, Dietitian, Social Workers, and Activities Coordinators.

The entire program cost may be covered by Medi-Cal Managed Care Health Plan. Participants who are not covered by Medi-Cal may enroll with customized charge as a (Private Pay).
